Step 1 - Lodge Application

1. Please complete all fields marked *.
2. An email confirming receipt of your application will be sent to you once this form has been submitted. Note: Seeley International can not process incomplete claim forms.

Step 2 – Proof of Purchase

1. When the confirmation email is received, please mail in the following:
   a. print out of the confirmation email
   b. a copy of your receipt as proof of product purchased during the promotional period. These documents must be received by 31st March 2010.
   
   Mail or fax these documents to: Seeley International Pty Ltd, Braemar Cashback Promotion, PO Box 140, Melrose Park, SA 5039, fax: 08 8374 3313 - by no later than 5pm (Adelaide time) on the 31st March 2010. Late applications will not be accepted and no extensions will be given under any circumstances.
   
   
2. You are advised to retain a duplicate copy of all your claim documents.

Terms & Conditions

1. This offer cannot be combined with any other offer.
2. Once your claim has been received and accepted by Seeley International, please allow up to 90 days for the delivery of your cheque by ordinary mail.
3. Seeley International will not accept liability for any claim forms that are lost or damaged.
4. Seeley International’s decision on all matters pertaining to this promotion (including eligibility to redeem the product and verification of claims) is final and binding and no correspondence will be entered into.
5. Seeley International reserves the right to change the closing date of the Cash Back Offer at its own discretion.
6. Employees of Seeley International Pty Ltd and participating dealers and related installers are not eligible for this offer.
